Client Interview Procedures

  1. Use an interpreter when a client has communication or language differences.
  2. Check that the information provided is correct:
    • name
    • contact details
    • date of birth
    • medicare details
    • previous history
    • emergency contact details
    • concession card details.
  3. Ensure interview with client takes place in interview or consulting room with door closed.
  4. Explain to the client their rights:
    • privacy
    • interpreter
    • patient advocate
    • review of service
    • safety.
  5. Explain to the client the steps involved in:
    • gathering information
    • discussing service options
    • self care approaches
    • record keeping processes.
  6. Photocopy and attach to file any relevant reports/documents.
  7. Record detailed notes of client history.
  8. Fill in a Referral From and place photocopy on file when referring client to other services.
  9. Consult with supervisor regarding clients who may:
    • be in "at risk situations"
    • mandatory reporting issues (child abuse)
    • indicators of suicide or self-harm issues.
  10. Follow safety procedures in event of:
    • an abusive or threatening client
    • a client displaying aggressive behaviour
    • a family member or other community member threatening you or a client.
  11. Note any follow up appointments necessary.
  12. Sign and date patient record.
  13. File and store patient records securely before seeing another client.
